18 years of age or older.
As a new client. You will receive a pharmacist assigned directly to you. The pharmacist will create an individualized report for you after
reviewing all over the counter and prescribed medications for:
-Errors, dosage appropriateness, Allergies, cost of medications, interactions, clinical appropriateness,insurance coverage and cost of medications.
- The pharmacist will speak with you gathering all information medical and personal that is important for your care.
- Speak to your provider on your behalf to get clarifications on any concerns that
He/she may find.
-Create a personalized and simple to follow report that brings together your total health profile in a way that makes sense for you.
- Answer any questions or concerns you may have about your report.
On month two and beyond as a returning client. We will continue to follow up on all:
-New labs, interchanges or substitutions.
-Monitor and explain what you can do to improve your care.
-Check up on your care
-Make changes to your report
-Answer any on going clinical questions within the scope of pharmacy practice.
-Consultation with an expert pharmacist.
-In depth review of the clients medications.
